Create an intake or consent form

Updated 2026-10-22 · 3 min read

Forms collect what you need from a client before their appointment: health questions, patch-test declarations, consent to treatment. Answers are saved to the client and the booking. Forms are available if your plan includes them.

Global questions and service forms#

Go to Settings → Forms. The page has two parts:

  • Global Questions: questions that appear on every booking, such as "How did you hear about us?". Open the drawer to edit labels in English and French, add options or hide a question.
  • Service-Specific Forms: forms that appear only for the services or categories you attach them to.

Build a form#

  1. Choose New form to start blank, or New from template to start from a ready-made form for your type of business.
  2. Enter a Form name and an optional Description.
  3. Under Fields, use Add field and pick a type: short or long text, checkbox, radio, select or multi-select, phone, email, number, date, consent, or a section heading to split long forms.
  4. Give each field a label in English and French and tick required where an answer is essential.
  5. Choose the Mode: Booking only (filled in while booking), Standalone (public link) (shared on its own link) or Both.
  6. Under Attach to (publishes live), pick the services or categories, then click Save & publish.

To collect a signature, turn on Require signature; see Collect consent signatures.

How the client gets the form#

  • While booking: in Booking only or Both mode, the questions appear on the details step for the attached services.
  • In the booking emails: in Standalone (public link) or Both mode, switch on Email this form with bookings. The confirmation and reminders then carry a link, and the answers are linked to the booking, see Send a form with the booking confirmation email.
  • On its own link: share the Public link shown for standalone forms, or open it on a tablet at the front desk.

To see each path from the client's side, read What clients see when you ask for a form.

Where answers go#

  • On the booking: open it from the calendar and read Form answers.
  • On the client: the Consent forms section of their record.
  • Per form: View submissions lists every response, with New, Viewed and Archived filters and Export CSV. Choice answers show the option label the client saw.

Answers marked sensitive are only visible to team members whose role allows it; see Team members, roles & permissions.
