Collect consent signatures
For treatments that need documented consent, ask the client to sign a form. They sign on screen and the signed form is stored against their record.
Add a signature to a form#
- Go to Settings → Forms and open the form (or create one, see Create an intake or consent form).
- Turn on Require signature.
- Write the Consent agreement. Use the language switcher in the field to write it in each language you serve. It is shown to the client right above the signature box.
- For a single agreement line inside the form, you can also add a field of type consent, which the client must tick to continue.
- Click Save & publish.
How clients sign#
- When booking online: if the form is attached to the service, the client reads the consent text and signs as part of booking.
- By email after booking: switch on Email this form with bookings and attach the form to the services. The confirmation and reminder emails carry a link, and the signed form is linked to that booking automatically, see Send a form with the booking confirmation email.
- Before or at the appointment: set the form Mode to Standalone (public link) or Both, then copy its Public link. Send it to the client, or open it on a tablet or phone at the front desk and hand it over.
Where to find signed forms#
Signed forms appear in the Consent forms section of the client's record, on the linked booking, and in the form's View submissions list. To see the signing step as the client does, read What clients see when you ask for a form.
Storage and privacy#
Signatures are private client data. They are stored securely and only open for signed-in team members with access to that client; a copied file link does not work for anyone else. See How Lorenly protects client data & photos.
