De Zorgcoach offers tailored support in Zaandam and provides outreach support in Noord-Holland, Zuid-Holland and Noordoost-Brabant. The coaches support people with skills, conversations with organisations, daytime activities, behaviour and independence.

Support

Support is tailored to the client’s wishes and goals. The coaches can also involve people around them and help with conversations at school, work and with organisations.

Approach

The client decides what help they want, when, where and with whom. De Zorgcoach provides outreach support, including early in the morning, in the evening and at weekends.
