Approach

• Follow-up conversations with guidance and coaching
• Written report to the referrer after the first and last consultation, and in between if needed

Locations

Consultation hours are in Geldrop, Heeze and Mierlo. Home visits are possible if someone cannot come to a location for medical reasons.
